POSITION TITLE: Clinical Pharmacist
PROGRAM: Post Concussion Management Program
REPORTS TO: Rehabilitation Services Manager - PCMP
LOCATION: Newton, Surrey
STATUS: Part Time, Permanent Employee Status (0.4 FTE | 15 hours/week) or Contractor Status
HOURS OF WORK: 15 hours/week | 2 days/week
Wage Range: $55.00 to $60.00 per hour (Employee Rates)
Contractor Rate: TBD
RESPONSIBILITIES
- Optimize medication treatment plans to minimize actual and potential adverse effects while maximizing benefits, supporting the best possible outcomes for clients’ functional ability and return to work
- Provide evidence-based recommendations and clinical insights to the team Physician and clients, particularly in complex cases, with a focus on improving overall health
- Educate and support clients in safely managing medications at work and home, promoting adherence and ensuring suitability for safety-sensitive roles
- Collaborate with clients and the treatment team to assess how medication changes impact functioning
- Review and assess medications to identify opportunities for optimization, with a focus on pain, headaches, mood, and sleep
- Reduce risks associated with current and potential treatments
- Educate clients to support understanding, cooperation, and adherence to medication management plans
- Work closely with the team Physician to develop and implement treatment plans, including titrations, tapers/weaning, and withdrawal management
- Provide ongoing weekly monitoring and reassessment to adjust plans, support functional goals, and enhance overall quality of life
QUALIFICATIONS
- Licensed Pharmacist in good standing with the College of Pharmacists of British Columbia
- Minimum of 2 years of experience working with patients with concussions or in a similar clinical setting
- Strong clinical knowledge with a commitment to patient-centered care
- Excellent communication, problem-solving, and organizational skills
- Ability to work independently and make sound clinical and operational decisions
- Comfortable working within a similar work setting such as the post-concussion, pain management or return to work program with a broad scope of responsibilities
