Overview
We are seeking a dedicated and experienced Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including pediatrics, senior care, and acute care environments. This role offers an opportunity to work with diverse patient populations, including those in nursing homes, assisted living facilities, hospitals, and home care settings. The Nurse Practitioner will collaborate with physicians and healthcare professionals to deliver high-quality medical services, utilizing advanced clinical skills and medical knowledge. Experience with electronic health records such as Epic and Cerner is preferred, along with proficiency in medical terminology and medication administration.
Duties
- Conduct thorough patient assessments and develop individualized treatment plans
- Diagnose and manage acute and chronic illnesses across different age groups, including pediatrics and seniors
- Administer medications and perform procedures such as phlebotomy and laboratory testing
- Provide pain management, dementia care, and case management services
- Collaborate with healthcare teams to coordinate patient care in various settings like hospitals, nursing homes, assisted living, and home care
- Document patient interactions accurately using electronic health records (Epic, Cerner)
- Educate patients and families on health conditions, treatment options, and preventive care strategies
- Monitor patient progress and adjust treatment plans accordingly
- Ensure compliance with healthcare regulations and safety protocols
Skills
- Proven experience working with children, seniors, or in hospital settings
- Strong background in nursing home experience, acute care, or home care environments
- Expertise in pain management, dementia care, case management, and laboratory procedures
- Proficiency in medical terminology, medication administration, and electronic health record systems (Epic, Cerner)
- Excellent patient care skills with a compassionate approach toward diverse populations
- Ability to handle complex cases involving pediatrics or assisted living residents
- Effective communication skills for collaborating with multidisciplinary teams
- Knowledge of medical office procedures and laboratory testing processes
- Experience in phlebotomy and laboratory work is a plus
