Job Title: Registered Nurse – Lung Health Program
Salary range: $38.69 - $41.05 per hour
Reports to: Clinical Director
Position Summary
The Registered Nurse - Lung Health Program provides comprehensive nursing care and education to patients with chronic respiratory conditions within a primary care setting. The role focuses on spirometry testing, lung health education, pulmonary rehabilitation, chronic disease management and patient self-management strategies in collaboration with the interdisciplinary team.
Responsibilities
- Provide patient education related to lung health, respiratory disease prevention, inhaler technique, smoking cessation, oxygen therapy and self-management strategies.
- Support and facilitate pulmonary rehabilitation programming, including monitoring patient progress and encouraging wellness goals.
- Conduct nursing assessments and develop individualized care plans for patients with chronic respiratory conditions such as COPD and asthma.
- Act as a clinical resource for respiratory health and chronic disease management, while monitoring patient symptoms, medication adherence, and treatment response in collaboration with primary care providers.
- Promote evidence-based chronic disease management and healthy lifestyle interventions.
- Maintain accurate and timely clinical documentation within the EMR according to professional standards.

Qualifications
- Bachelor of Science in Nursing (BSN).
- Current registration and in good standing with the College of Nurses of Ontario (CNO).
- Minimum of 3 years of nursing experience, preferably in respiratory, chronic disease management, or primary care settings.
- Experience performing spirometry testing considered an asset.
- Certified Respiratory Educator (CRE) considered an asset.
- Certification or training in pulmonary rehabilitation considered an asset.

Work Schedule:
This is a full-time, permanent position with a standard 37.5-hour work week, Monday to Friday.
