Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for a permanent full-time position. The ideal candidate will provide high-quality patient care across various healthcare settings, including nursing homes, hospitals, assisted living facilities, and home care environments. Responsibilities
- Administer medications and treatments accurately following physician orders and established protocols
- Monitor patient vital signs and assess patient conditions regularly
- Document patient information accurately using electronic health records such as Epic or Cerner systems
- Assist with patient admissions, discharges, and transfers as needed
- Provide compassionate patient care tailored to individual needs, including dementia, hospice, pediatric, or senior care
- Collaborate with interdisciplinary teams to develop and implement personalized care plans
- Perform phlebotomy procedures and laboratory testing when required
- Educate patients and families about health management and medication usage
- Ensure compliance with healthcare regulations and safety standards
- Support case management activities to coordinate ongoing patient care
Requirements
- Valid LPN license in the state of practice
- Prior experience in nursing home, hospital, acute care, assisted living, or home care settings preferred
- Knowledge of medical terminology, medication administration, and patient care procedures
- Experience with electronic health record systems such as Epic or Cerner is a plus
- Strong communication skills and ability to work effectively within a team
- Ability to handle multiple responsibilities with professionalism and compassion
- Experience working with diverse populations including children, seniors, or hospice patients is advantageous
- Certification in phlebotomy or laboratory procedures is desirable but not required
- Ability to work flexible hours as needed; some positions may require working weekends or holidays depending on the facility’s needs
