About the Role
As our Lead Personal Support Worker, you'll help ensure clients continue to receive exceptional care when it's needed most.
Rather than carrying a regular client caseload, you'll respond to urgent client needs, provide coverage for unfilled visits, support the Community Nurse Manager, and mentor fellow caregivers to help maintain the highest standards of care across the branch.
This is a full-time (40 hours/week) position. Flexibility is required to respond to urgent operational needs and help ensure continuity of care.
Key Responsibilities
Support Clients
- Respond to urgent client needs and staffing gaps
- Support clients with complex care needs
- Ensure care plans and service standards are consistently delivered
- Model exceptional person-centred care
Support Operations
- Assist the Community Nurse Manager with new client starts, supervisory visits, and care transitions
- Help resolve day-to-day operational challenges
- Support quality improvement initiatives
Support Caregivers
- Welcome and mentor new Personal Support Workers
- Provide hands-on coaching in clients' homes
- Share best practices and promote CareHop's standards of care
Qualifications
- Personal Support Worker Certificate from a recognized program
- Minimum 5 years of PSW experience, including at least 2 years in community home care
- Strong communication, documentation, and interpersonal skills
- Experience with electronic documentation (AlayaCare is an asset)
- Valid Ontario driver's licence and reliable vehicle
