The Sagkeeng Health Centre seeks an experienced, self-directed, and highly motivated Public Health Nurse Practitioner Manager who will manage, plan, implement, deliver and evaluate community/public health programs and services for Sagkeeng First Nation community members while working in the community. This job is located at the Sagkeeng Health Centre in Fort Alexander and is in-person working only.
The successful candidate will ensure nursing care and practice standards are being implemented in the delivery of programs and services. These programs include infant and school-based immunizations, adult immunizations, pre and post-natal care, rabies prevention, STBBI monitoring, and community health prevention and education. This position will report to the Director of Health and work closely within the Community Health team to organize and deliver care for individuals and families living in Sagkeeng First Nation.
Responsibilities
- · Provide leadership and opportunities to expand the Public Health Nurses knowledge and abilities in the community by setting key performance indicators to staffing and help lead them through change.
· Provide public health services to members in the community, including health promotion activities and education
· Nurse Practitioner duties; see patients / write prescriptions / medication refills order lab tests / diagnose medical conditions / referrals to specialists and public health requests as needed
· Deliver immunization clinics such as baby clinics, school clinics, flu clinics, and updating adult immunizations
· TB monitoring, education, testing & treatment as required
· Monitoring of communicable diseases, contact tracing and treatment
· Deliver the rabies prevention program as required
· Complete reports on various public health programs such as immunization stats, breastfeeding, and education events
· Support breastfeeding initiatives pre and post-natal
· Continue and maintain current knowledge in nursing practice by reviewing literature, evaluating one’s own nursing practice and participating in relevant professional development opportunities
· Provide a linkage between all community organizations, hospital, physicians, diabetes education program and other external health partners
Qualifications
- · Current Registration and good standing with College of Registered Nurses of Manitoba (CRNM),
· Graduate of an accredited school of nursing with Bachelor of Nursing degree or Registered Nurse with significant relevant work experience,
· Knowledge of the nursing process and its application, including assessment implementation, and evaluation of nursing care,
· Demonstrated ability to assist clients and families to access care that is delivered with integrity, respect and compassion,
· Maintain confidentiality and privacy of client files and personal health information,
· High level of critical thinking and flexibility in delivering care that follows best care practices,
· Excellent verbal and written communications skills,
· Exceptional interpersonal communication skills and ability to handle difficult and challenging situations,
· Knowledge of First Nations culture and awareness of the social determinants of health and social challenges within First Nations communities is essential,
· Proficiency in Microsoft Office and Excel programs,
· Must have a valid driver’s License, valid access to a reliable vehicle and clear driving record.
