Summary of Duties:
Reporting to the Community Inclusion & Outreach Manager, the Community Inclusion Support Worker (CISW) supports adults with developmental disabilities who are eligible for Community Living B.C. (CLBC) services in a respectful, person-centered, culturally grounded manner. The CISW assists individuals with activities of daily living, life skills, personal care, health and wellness, community participation, and recreational activities while promoting independence, dignity, choice, and community belonging.
The CISW works closely with the Community Inclusion – Community Living B.C. Coordinator, families, caregivers, Lilwat Nation departments, and external professionals to support coordinated service delivery. The CISW follows all Lilwat Nation policies and procedures and applicable CLBC standards, including the Adult Guardianship Policy, Behavioural Support and Safety Planning Policy and Guidelines, Criminal Record Check Policy, and Critical Incident Policy.
Key Deliverables and Expectations:

Demonstrate and promote a person-centered approach by supporting individuals to participate in decisions regarding their own lives.

Work with the Community Inclusion (CLBC) Coordinator on case management related tasks, including daily/ weekly log of activities.

Establish and maintain professional, appropriate support relationships with the individual that nurture self-determination and personal growth.

Support individuals in maintaining meaningful connections with friends, family, elders, advocates, and community support—according to the person’s wishes and in a way that reflects their identity and community belonging, while maintaining clear professional boundaries.

Participate in the planning, implementation, review, and adjustment of individualized support plans with the Manager based on each person's strengths, interests, needs, and goals.

Assist individuals with activities of daily living, including housekeeping, shopping, hygiene, meal preparation, budgeting, and other tasks, as required.

Support individuals in following health, nutrition, and dietary plans established by physicians, health professionals, caregivers, or individualized support plans, including participation in wellness activities that support their goals.

Support individuals in building independence by utilizing community food services, including the Community Food Centre (such as market greens, breakfast, and lunch programs) and Food Bank services as needed.

Maintain communication with participants, families, caregivers, and service providers regarding appointments and service needs.

Assist participants in filling out forms, either electronically or on paper.

Assist with accounting activities, including preparing and submitting payroll information, mileage and expense forms, petty cash, purchase orders, and receipts, as needed.

Maintain high standards of confidentiality and professionalism in all community interactions.

Perform other related duties as required.
Experience and Attributes:

High school diploma or equivalent.

1–2 years of experience in community home care, disability support services, community inclusion, or special needs support services.

Community Support Worker Certificate or equivalent education and/or training is an asset.

Must have a valid Current Standard First Aid and CPR certification and must be obtained and maintained throughout employment.

Willing to work with people with diverse abilities and demonstrated personal suitability to promote dignity, independence, individuality, and inclusion.

Motivated self-starter with strong written and verbal communication skills.

Proficiency in Microsoft Office applications, including Word and Excel.

Culturally respectful and professional in all interactions with clients, staff, and community members.

Ability to work independently in clients’ homes and as an integral member of multidisciplinary teams.

Strong organizational, time management, and prioritization skills, with the ability to manage multiple tasks, meet deadlines, and maintain confidentiality.

Knowledge of and genuine interest in Lil’wat Nation territory, culture, and history is an asset.

Must have a valid BC Class 5 Driver’s License and access to a reliable vehicle.

Willing to obtain a BC Class 4 Unrestricted Driver's License within three (3) months of employment, unless otherwise approved.

Must be able to provide a clear Criminal Record Check including Vulnerable Sector screening.
