As a member of the Youth Psychiatry Program, you will use best practices to provide person centred Social Work services to transitional-aged youth and their families. This includes assessment, therapeutic intervention and discharge planning; in collaboration with the clinical team, client, families, and hospital and community partners.
Duties:

Provide assessment, short-term goal-focused treatment, and support to youth, families, and caregivers.

Deliver individual, family, and group therapy in line with best practice standards.

Conduct risk assessments and develop crisis intervention plans for high-risk clients (self-harm/suicide).

Engage clients and families in care plans and risk reduction strategies with the interdisciplinary team.

Advocate for clients and connect them to appropriate services and community resources.

Collaborate and liaise with internal teams and external partners.

Complete timely bio-psycho-social assessments, treatment plans, and discharge summaries.

Maintain accurate clinical documentation in accordance with hospital, college, and legislative standards.

Track workload metrics and support data collection, research, and program evaluation initiatives.

Participate in teaching, supervision of Social Work students, and professional development activities.

Attend peer consultation groups, discipline meetings, and contribute to committees/task groups.

Maintain up-to-date knowledge of policies, legislation, and professional standards.

Deliver educational sessions to community groups as required.

Work flexible hours (days/evenings) and adhere to health, safety, and organizational policies.

Qualifications:

Master’s degree in social work
-
Registration and in good standing with the Ontario College of Social Workers and Social Service Workers

Minimum of three (3) years clinical experience in a mental health setting

Experience working with youth and families in a mental health setting

Knowledge and experience in concurrent disorders, mood and anxiety, trauma and evidence-based treatments.
- Excellent organizational skills and the ability to work effectively in a fast-paced interprofessional clinical environment are essential.
- Demonstrated skills in psychotherapy, including CBT, DBT, or other relevant modalities; certification preferred.
- Understanding of early psychosis and youth substance use in individual and group settings.

Excellent knowledge of community resources related to the needs of youth and families.

Knowledge of recovery-based programs.

Knowledge of relevant legislation (e.g., MHA, PHIPA, Health Care Consent Act).

Ability to manage and determine priorities with a diverse case load.

Proficient in electronic health record systems for documentation and navigation.

Valid driver’s license and access to reliable vehicle.
-
English level A- is mandatory in oral comprehension, expression, reading, and writing. Bilingual (French/English) is considered an asset.
Salary Range:
44.67 to 57.90 per hour
