Strathcona Long Term Care in Mount Forest is seeking a qualified, dynamic Social Service Worker to join our team.
Position Summary
The Resident Services Coordinator supports high-quality, resident-centered care in accordance with the Fixing Long-Term Care Act, 2021 and O. Reg. 246/22. This role ensures residents receive services that reflect their needs, preferences, and rights, while upholding the Resident Bill of Rights and the organization’s mission, vision, and values.
Key Responsibilities
- Coordinate admissions, including tours, application reviews, and admission documentation
- Collaborate with DOC/ADOC on admission decisions and care planning
- Support new resident transitions, including room setup and initial care coordination
- Schedule, attend, and document multidisciplinary care conferences (MDCs); may chair meetings
- Act as a liaison for residents and families; assist with concerns and follow-ups as directed
- Assist resident with financial matters related to transition of benefits, rate reduction applications, sorting out POA and other financial affairs
- Support mobility equipment tracking and ensure timely completion of assessments
- Participate in Family Council, quality improvement, and risk management activities
- Liaison with community agencies to arrange and co-ordianate services for residents as required
- Facilitate and co-ordinate transition to other care settings as applicable
- Assist with community engagement and marketing initiatives
- Support residents and families in the Long-Term Care setting
- Mentor staff and promote a positive, resident-focused culture
- Perform other duties as assigned

Pay: $26.00-$30.00 per hour
