GENERAL SUMMARY
Reporting to the Clinical Director, the Social Worker provides assessment, counselling, case management, crisis intervention, and discharge planning services to clients participating in Brentwood's residential addiction treatment programs. The Social Worker works collaboratively with counsellors, medical staff, peer support workers, and community partners to support clients in achieving sustainable recovery.
ORGANIZATIONAL DUTIES AND RESPONSIBILITIES:

KEY RESPONSIBLITIES
· Conduct comprehensive psychosocial assessments.
· Develop individualized treatment and discharge plans in collaboration with clients and the interdisciplinary team.
· Provide counsel using evidence-based and trauma-informed approaches.
· Support clients experiencing co-occurring mental health and substance use challenges.
· Complete risk assessments and crisis intervention when required.
· Advocate for clients while promoting dignity, respect, and client-centred care.
· Maintain accurate and timely clinical documentation in accordance with professional standards.
· Participate in clinical case conferences, team meetings, and continuous quality improvement initiatives.
· Foster collaborative relationships with community agencies and referral partners.
· Maintain confidentiality and practice in accordance with applicable legislation and ethical standards.
QUALIFICATIONS
· Bachelor of Social Work (BSW) required; Master of Social Work (MSW) considered an asset.
· Current registration in good standing with the Ontario College of Social Workers and Social Service Workers.
· Minimum two years of clinical experience, preferably in addictions, mental health, residential treatment, or community health.
· Knowledge of addiction, recovery principles, trauma-informed care, and concurrent disorders.
· Experience providing crisis intervention and case management.
· Strong assessment, counselling, communication, and interpersonal skills.
· Ability to work effectively within a multidisciplinary team.
· Excellent organizational and documentation skills.
ADDITIONAL DETAILS
· Temporary, full-time (37.5 hours weekly), hourly position
· Hourly rate range - $24.76 – $30.26
· Police clearance with vulnerable sector check required
