Position Summary
The Youth Case Manager will provide intensive case management, support, advocacy, and service coordination for Black and racialized youth aged 12–17 who are involved in the criminal justice system and/or are being dealt with under the Youth Criminal Justice Act (YCJA).
Working within a trauma-informed, culturally responsive, and restorative justice framework, the Youth Case Manager will support youth who are gang-involved, violent offenders, on probation, in diversion programs, or involved in pre- or post-trial processes. The successful candidate will work closely with youth, families, schools, probation officers, police services, courts, Children's Aid Society, and community partners to facilitate rehabilitation, accountability, reintegration, and positive youth development.
The Youth Case Manager will play a critical role in helping youth address barriers related to trauma, mental health, education, employment, family conflict, community violence, and systemic inequities while supporting them in developing life skills, achieving personal goals, and reducing their risk of re-offending.
Key Responsibilities
Case Management & Youth Support
- Conduct comprehensive intake assessments and develop individualized case management plans for participants.
- Provide ongoing one-on-one case management and support services to youth involved in the justice system.
- Monitor participant progress and regularly update service plans, goals, and intervention strategies.
- Support youth in identifying and addressing barriers related to education, employment, housing, family relationships, mental health, substance use, and community safety.
- Maintain regular contact with participants and provide crisis intervention, advocacy, and referrals as required.
- Support youth in fulfilling court-ordered conditions, probation requirements, restorative justice commitments, and community service obligations where applicable.
- Maintain accurate and timely case notes, participant records, assessments, and documentation.
Restorative Justice & Reintegration
- Support youth participation in restorative justice circles, peer accountability groups, conflict resolution activities, and community service initiatives.
- Assist participants in developing accountability plans and strategies to repair harm caused to victims, families, and communities.
- Facilitate personal reflection, goal setting, and positive decision-making processes.
- Promote pro-social behaviours, emotional regulation, conflict resolution, and responsible citizenship.
- Support youth transitioning back into school, employment, training, and community settings.
Family & Community Engagement
- Build trusting relationships with youth and their families through consistent engagement and support.
- Conduct home visits and community-based outreach as required.
- Support family mediation, conflict resolution, and parent engagement activities.
- Connect families to community resources, supports, and services that strengthen family stability and youth outcomes.
- Advocate on behalf of youth and families when navigating community, educational, justice, and social service systems.
Service Coordination & Partnership Development
- Coordinate services with probation officers, police services, schools, courts, Children's Aid Society, mental health providers, community agencies, and other stakeholders.
- Participate in case conferences, multidisciplinary meetings, and service planning discussions.
- Develop and maintain strong working relationships with community partners to ensure coordinated support for participants.
- Facilitate referrals and ensure youth successfully access appropriate services and supports.
Program Administration & Reporting
- Maintain accurate participant files and case management records in accordance with organizational policies and confidentiality requirements.
- Collect participant data, outcome measurements, and program evaluation information.
- Prepare reports, progress summaries, and documentation required by funders and management.
- Participate in staff meetings, training sessions, and continuous quality improvement activities.
- Ensure compliance with all organizational policies, funder requirements, and professional standards.
Qualifications
- Diploma or degree in Social Work, Child and Youth Care, Criminology, Psychology, Mental Health and Addictions, Community Justice, Social Services, Community Development, or a related field.
- Minimum three (3) years of experience providing case management, youth outreach, counselling support, or community-based services.
- Experience working with justice-involved youth, high-risk youth, gang-involved youth, or youth facing complex barriers.
- Strong knowledge of youth development, trauma-informed care, restorative justice, violence prevention, and rehabilitation approaches.
- Experience conducting assessments, developing service plans, and managing participant caseloads.
- Excellent crisis intervention, conflict resolution, and problem-solving skills.
- Strong verbal, written, interpersonal, and documentation skills.
- Ability to build rapport and establish trust with youth, families, and community stakeholders.
- Proficiency in Microsoft Office Suite and electronic case management systems.
Preferred Knowledge & Experience
- Knowledge of the Youth Criminal Justice Act (YCJA), youth justice processes, probation services, diversion programs, and restorative justice practices.
- Understanding of gang involvement, community violence, adverse childhood experiences, and youth offending risk factors.
- Familiarity with Scarborough communities and community-based resources available to youth and families.
- Experience facilitating youth groups, workshops, mentorship activities, or restorative justice circles.
- Knowledge of mental health, addictions, housing, employment, and educational support systems.

- Demonstrated understanding of the unique experiences and challenges faced by Black, racialized, newcomer, and underserved youth.
- Strong knowledge of anti-Black racism, systemic barriers, trauma, community violence, and their impact on youth development.
- Commitment to equity, inclusion, anti-oppression, and culturally responsive service delivery.
- Ability to work flexible hours, including evenings and occasional weekends.
- Valid Ontario driver's licence and reliable access to transportation considered an asset.
- Ability to conduct community visits, school visits, home visits, and attend court-related meetings when required.
- Current Standard First Aid/CPR certification is considered an asset.
- Successful Vulnerable Sector Check required upon hire.
Pay: $28.00 per hour
