Thresholds Homes and Supports Inc. is a community-based agency that promotes recovery and improved quality of life for people who are experiencing mental health and addiction issues by providing affordable housing and individualized, flexible support services through our wide-ranging programs offered throughout Waterloo Region and Wellington County. SUMMARY OF POSITION

The Social Worker is responsible for conducting comprehensive psychosocial and mental status assessments and providing intensive, recovery-oriented support to clients. The incumbent takes a lead role in delivering treatment, rehabilitation, counselling, coaching, and individualized supports based on collaboratively developed personal support plans. Acting as a primary case manager for assigned clients and providing backup case management to others, the position supports continuity of care. The role also involves developing and facilitating team activities that support families, assisting clients in identifying and strengthening interpersonal and life skills, and linking clients to appropriate community resources while providing consultation and support to other team members.

HOURS OF WORK

The Social Worker will work a thirty-five (35) hour week, the hours being flexible to the needs of the program.

SALARY RANGE: $34.02 to $41.35 per hour

ESSENTIAL QUALIFICATIONS

Bachelor or Master’s Degree in Social Work from an accredited university.
Registration with the Ontario College of Social Workers and Social Service Workers.
Three to five years’ experience in a community mental health setting working with adults recovering from serious mental illness.
Certification in CPR, non-violent crisis intervention and in suicide risk assessment preferred.
Knowledge of mental health resources within the Waterloo Region.
Demonstrated skill in applying the principles of PSR, Recovery and case management.
Working knowledge of the Mental Health Act and other relevant legislation.
Specific training in family intervention approaches.
Experience using a harm reduction approach and motivational interviewing.
Experience working with individuals who have both mental health challenges and acute housing instability.
Experience in supporting individual vocational programing and collaboration with community resources.
Must have a valid driver’s license, relevant insurance and use of a reliable vehicle.
Proficient in MS Office Suite, PC Skills, and documentation.
Satisfactory police vulnerable sector check.
