Job Description

Position Summary:

The Coordinator, Emergency Department (ED) and Substance Use Disorders Integrated Care Pathway (SUD ICP) supports the SUD ICP and provides navigation and care coordination across ED, inpatient, outpatient, and community services. The role provides hospital-embedded navigation and care coordination for individuals with substance use disorders and co-occurring mental health and social needs, supporting timely access, safe transitions, and continuity of care across the Emergency Department, inpatient, outpatient, and community settings. Care delivery is grounded in a harm-reduction, trauma-informed, patient-centred, and equity-based approach.

Qualifications:

3 year Community College Diploma in Nursing, Social Work, Occupational Therapy, Physiotherapy, Social Services, Human Services or Psychology required.
Current registration in good standing with a related regulated health professional College
3-5 years of relevant experience in mental health, addictions, care coordination, discharge planning, or community navigation
3-5 years of experience in hospital, community health, or integrated care settings preferred.
Strong grounding in cultural safety, harm reduction, trauma-informed care, and patient-centred practice. [SA1.1]
Knowledge of substance use, mental health, withdrawal, crisis response, and social determinants of health.
Ability to work effectively with structurally vulnerable populations, including individuals experiencing homelessness, justice involvement, or systemic barriers.
Excellent communication, relationship-building, advocacy, and system coordination skills.
Ability to work independently in a fast-paced hospital environment while collaborating across teams.
Demonstrated ability to build effective working relationships across hospital and community partners.
Strong organizational skills and ability to manage competing priorities in a dynamic clinical environment.
Embodies cultural safety through lived experience and/or meaningful community connection, and demonstrates the ability to act in ways that uphold dignity, respect, and equity for Indigenous, newcomer, and marginalized communities.

Responsibilities:

Identify and engage patients with complex substance-use needs and provide brief assessment and motivational engagement.
Support initiation and continuity of evidence-based treatments, including OAT/MAT where appropriate, in collaboration with the interprofessional team.
Act as a liaison between hospital teams and community partners, advocating for culturally safe, non-stigmatizing care.
Coordinate safe discharge planning, including referrals to community and social supports.
Maintain clear documentation and contribute to data collection and quality improvement activities related to the pathway.
Participate in SUD ICP working groups and related implementation activities to refine and improve pathway processes.
Support a workplace that embraces diversity, encourages teamwork, and complies with all applicable regulatory and legislative requirements.
