Santé Circle Health is seeking a compassionate, organized, and detail-oriented Abilities Case Manager to join our multidisciplinary team.
This role is responsible for managing a dedicated caseload that may include short-term disability, salary continuance, workplace accommodation, and return-to-work matters. The successful candidate will complete intake and file reviews, assess medical and functional information, support appropriate disability and accommodation decisions, and develop return-to-work plans in collaboration with employees, employers, healthcare providers, and other stakeholders.
The Abilities Case Manager will provide clear, timely, and evidence-informed case management services while maintaining accurate documentation, meeting contractual service standards, and supporting positive and sustainable workplace outcomes.
Key Responsibilities:
- Manage a dedicated caseload, including short-term disability, salary continuance, workplace accommodation, and return-to-work files, while meeting applicable service-level requirements.
- Conduct intake assessments and comprehensive file reviews to understand medical information, functional abilities, workplace requirements, and barriers to recovery or return to work.
- Assess eligibility for benefits, accommodations, or other applicable supports in accordance with client requirements and established processes.
- Make evidence-informed case management decisions and communicate outcomes clearly, professionally, and compassionately.
- Develop, implement, and monitor individualized return-to-work and accommodation plans in collaboration with employees, managers, healthcare providers, and other stakeholders.
- Identify barriers to recovery, accommodation, and return to work and recommend practical strategies to support resolution.
- Communicate with healthcare providers to obtain clarification or additional medical and functional information where required.
- Prepare case summaries, correspondence, decision letters, reports, and other documentation for internal and external stakeholders.
- Maintain complete, timely, and accurate case documentation while ensuring confidentiality and appropriate handling of personal and medical information.
- Provide employees and other stakeholders with guidance regarding disability management, accommodation, and return-to-work processes.
- Participate in case management meetings to review complex files, barriers, progress, and proposed solutions with internal teams and clients.
- Review program and contract data and identify trends or opportunities to improve accommodation, salary continuance, sick leave, disability management, or legislative leave programs.
- Collaborate with members of the multidisciplinary team and escalate complex or sensitive matters where appropriate.

Qualifications:
- Regulated Healthcare Professional in good standing with the applicable regulatory College. Relevant professional backgrounds may include:
- - Registered Kinesiologist
- - Physiotherapist
- - Occupational Therapist
- - Other regulated healthcare professionals with relevant disability or case management experience
- Minimum of 3 years of relevant experience in disability management, abilities management, case management, occupational rehabilitation, workplace accommodation, or return-to-work planning.
- Experience managing occupational and/or non-occupational injury and illness cases.
- Working knowledge of disability management principles, medical terminology, functional abilities, workplace accommodation, and return-to-work practices.
- Working knowledge of applicable Ontario legislation, including the Ontario Human Rights Code and Employment Standards Act.
- Experience interpreting medical and functional information and applying it to workplace accommodation and return-to-work planning.
This position is compensated on an hourly basis. The expected annual earrings for this role are $62,400, based on full-time hours.
