McMan Youth, Family and Community Services Association

McMan is a not-for-profit agency that has been operating since 1975. McMan provides a range of comprehensive programs across Alberta. Earning a sound reputation from success stories and positive relationships, McMan has become well known for being a responsive, highly competent, and innovative partner in service delivery. A diverse range of programs and services are accessible through local offices across Alberta to anyone who needs help. We believe that help from someone who cares should never be out of reach.

Our Coordinated Entry program in Red Deer is seeking a Permanent Full Time Coordinated Entry Outreach Facilitator

The Coordinated Entry program is for youths between 15 and 24 who have been homeless for three months or have a history of episodic homelessness that spans a significant portion of the last few years. Referrals are accepted based on: youth without a permanent place of residence; living on the street, in shelters or places not intended or suitable for permanent residency; “couch surfing”, or living off others for short periods of time. An interview with the youth/family being served is also part of the process. Prioritization includes reviewing all available information, discussions with referral sources and information obtained for admission and screening reports

Minimum Qualifications and Requirements:

Diploma at minimum, preferred degree directly related to Human Services
Required 1 year of frontline human services experience
Proof of Membership in Professional Association (when applicable)
Knowledge and understanding of SPDAT considered an asset
Basic First Aid Level C with adult, child, and infant CPR &AED
Suicide Intervention Training & De-Escalation Training
Intervention Record & Criminal Record Checks with the vulnerable sector
Class 5 drivers license, clean drivers abstract and reliable vehicle for client transportation required.

Responsibilities include but are not limited to:

Develop relationships with homeless youth.
Work in partnership with Adult Coordinated Entry
Support the warm transfer of youth from intake to housing
Access appropriate community support for youth
Work with external organizations to facilitate transitions and wraparound services
Work to promote the program within the community

Wage Range: $23.46 - $26.24 per hour. Plus, Travel Reimbursement at $0.57/KM.

Hours of work: 40 hours per week. Monday through Friday.
