JOB SUMMARY
Siloam Mission’s Medical Transitional Care Unit (MTCU) is a compassionate 32-bed facility supporting individuals experiencing homelessness who have medical needs that do not require hospitalization but exceed what can be safely managed in shelter or the community. The MTCU provides time-limited transitional housing integrated with medical, mental health, and social supports to improve health stability and housing outcomes.
Guided by person-centered, strengths-based, trauma-informed practice, and rooted in harm reduction and Housing First principles, the MTCU operates through a multidisciplinary team approach.
The MTCU Registered Psychiatric Nurse (RPN), Registered Nurse (RN), or Licensed Practical Nurse (LPN) with extensive mental health experience provides direct, front-line nursing care to residents with complex mental health, substance use, and physical health needs. Residents may be actively using substances, experiencing acute or chronic mental illness, and exhibiting challenging or unpredictable behaviors. This role is essential in supporting resident safety, stabilization, emotional regulation, and engagement in care, while working closely with medical, housing, and social service partners to prevent unnecessary emergency department visits and hospital admissions.
KEY DUTIES & RESPONSIBILITIES

Provide direct nursing care and mental health support to residents in the MTCU within a transitional housing environment

Conduct ongoing mental health and behavioral assessments, recognizing changes in mood, cognition, behavior, and risk, and escalating concerns appropriately
-
Support residents experiencing acute mental health distress, agitation, psychosis, withdrawal symptoms, or emotional dysregulation, using trauma-informed and de-escalation techniques

Administer medications as prescribed and within scope of practice, including oral, injectable, and PRN medications, and monitor for therapeutic response and adverse effects

Deliver nursing interventions within scope of practice, including:

Wound care and dressing changes

Injections and medication administration
-
Monitoring vital signs and physical health status
-
Supporting IV therapies as directed
-
Responding to urgent medical or mental health situations, including Basic Life Support (BLS)
-
Work within a harm reduction framework, supporting residents who are actively using substances and providing education on safer use, overdose prevention, and infection control
-
Collaborate closely with the Nurse Practitioner, physicians, and interdisciplinary team members (e.g., healthcare attendants, case managers, occupational therapists, peer support workers)

Support residents in developing coping strategies, emotional regulation skills, and engagement with care
-
Participate in care planning, case conferencing, and coordination with hospitals, crisis services, and community mental health providers

Maintain accurate, timely, and professional documentation in accordance with regulatory and organizational standards

Contribute to a safe, respectful, and therapeutic environment within the MTCU

Participate in quality improvement initiatives, training, and program development as required
EDUCATION & EXPERIENCE
-
Registered Psychiatric Nurse (RPN) licensed in Manitoba OR
-
Registered Nurse (RN) licensed in Manitoba with extensive mental health experience OR
-
Licensed Practical Nurse (LPN) licensed in Manitoba with extensive mental health experience

Active registration with the appropriate regulatory college
-
Current Basic Life Support (BLS) certification (additional mental health, crisis response, or addictions training an asset)

Minimum of 2 years of relevant clinical experience preferred, ideally in one or more of the following settings:

Inpatient or community mental health
-
Addictions services
-
Emergency or crisis response
-
Health care within an emergency shelter, or transitional housing
-
Experience working with underserved or marginalized populations strongly preferred
KNOWLEDGE/SKILLS/BEHAVIORS

Strong commitment to harm reduction, trauma-informed, and recovery-oriented practice

Comfort and competence working with individuals experiencing mental illness, substance use, and homelessness

Ability to assess and respond to challenging behaviors, emotional distress, and crisis situations calmly and effectively

Skilled in de-escalation, boundary setting, and therapeutic communication
-
Sound clinical judgment and ability to prioritize care within scope of practice
-
Ability to work both independently and collaboratively within an interdisciplinary team
-
Flexibility, adaptability, and resilience in emotionally demanding work
-
Strong organizational, documentation, and time-management skills
-
Understanding of homelessness, poverty, systemic barriers, and social determinants of health
-
Knowledge of Indigenous history, colonization, and culturally safe care practices
-
Ability to work respectfully with and value diverse staff, including those with lived experience

Proficiency in Microsoft Word, Excel, and Outlook
RELATIONSHIP
-
Reports to Manager, Medical Transitional Care Unit

Provides nursing and mental health support to residents of the MTCU
-
Member of an interdisciplinary healthcare and housing support team

Collaborates with Nurse Practitioner, health care attendants, housing staff, and community partners

Acts as a liaison with hospitals, crisis services, and community mental health providers

Contributes information and reports to leadership as required
WORKING CONDITION
-
Hours: Mon-Fri 8:00 a.m. – 4:30 p.m.; some evenings and weekends may be required
-
Work occurs primarily within a transitional housing facility, including resident rooms
-
May be required to work independently with residents in their living environment
-
May be required to lift up to 50lbs, repetitive motions (bending, twisting, lifting, etc.)

Regular exposure to substance use, mental health crises, complex medical needs, and emotionally demanding situations
-
Potential exposure to unpredictable or high-risk behaviors, requiring situational awareness and adherence to safety protocols
WORKPLACE SAFETY AND HEALTH
The incumbent contributes to making the organization safe for clients and staff and recognizes the importance of reporting unsafe situations and participating in follow-up reviews as a learning opportunity.

Provides a safe environment by ensuring adherence to Workplace Safety and Health Regulations and Policies, Infection Control Guidelines, WHMIS and Safe Work Procedures

Immediately investigates and recommends corrective action on any unsafe acts, work conditions, incidents, near misses, injuries or illnesses

Demonstrates understanding of role and responsibilities in fire prevention and disaster preparedness and participates in safety and health training programs including the facility’s Fire, Disaster and Evacuation Plan

Supports an environment which avoids, prevents and corrects all activities or actions which may result in an adverse outcome in the delivery of services. Community Member safety is a standing item for all individual and departmental meetings

Salary: $71,905.60 - $102,128.00
