Your Opportunity:

As a Primary Care Clinical Pharmacist, you will be part of the team in the fast-paced practices of family physicians. You will work collaboratively with other health professionals to provide comprehensive, accessible primary health care, honouring patient choice in how best care will be delivered (in-person, telephone, video). We are currently seeking a Clinical Pharmacist to join our dynamic interdisciplinary team. This role supports patients and families across our community through assessment, intervention, education, and collaboration with healthcare partners.

Pharmacists in the Hamilton Family Health Team (HFHT) aim to improve drug therapy outcomes using a collaborative care model that integrates pharmacists into the primary care team.

Roles and Responsibilities:

Collaborate with interprofessional primary care team to match patient needs with available primary care capacity.

Strengthen links between all levels of the health care system.

Contribute to improved access and continuity of care across local healthcare services.

Support the effective use of resources to meet service demands.

Participate in quality improvement initiatives and practice enhancements including academic detailing and evidence informed drug prescribing in primary care.

Ensure services reflect current best practices and participate with HFHT on program evaluation and innovation model development.

Ensure compliance with Ontario College of Pharmacists professional standards.

Promote a safe, inclusive, and respectful care environment

Foster a culture of patient engagement and shared decision-making

Model the HFHT’s values of collaboration, inclusivity, and excellence

Support and collaborate with primary care team to maximize team effectiveness

Champion equity, diversity, and psychological safety in the workplace

Serve as a leader, advocate and ambassador of pharmacists in primary care

Integrate into various family medicine group practices by collaborating with physicians, other health care professionals and patients/families to promote optimal, patient-centred care.

Provide individual patient assessments to identify, prevent and resolve drug-related problems. Assessments include conducting medication histories, identifying problems, developing and monitoring care plans and then communicating the plan to the patient and interdisciplinary team.

To improve access to pharmacy services in primary care

To provide recommendations, adjust and prescribe medications as outlined in Ontario College of Pharmacists scope of practice, and follow up with patient care plans

Utilize medical directives for expanded scope activities (e.g. Ordering lab work, initiating pharmacotherapy outside of scope of practice)

Provide pharmaceutical consultation, pharmacotherapy monitoring, education/teaching and drug information.

Support health promotion, disease prevention and chronic disease management.

To provide education and mentorship to primary care team members and learners to support medication management

Develop quality improvement initiatives and practice enhancements based on the needs of the HFHT

Qualifications and Skills:

Bachelor of Science degree in pharmacy or PharmD from a recognized university

Registration in good standing in with the Ontario College of Pharmacists (Part A)

Completed a community or hospital residency or have equivalent clinical experience in either setting*

Able to work in an interdisciplinary team environment

Able to prioritize and be flexible in a very active work environment

Demonstrated expertise in practicing pharmaceutical care, problem solving and critical thinking

In addition, it is preferred that you have:

Minimum 2 years of relevant experience in a primary care setting

Additional training or credentialing in pharmacotherapy, evidence-based medicine or pharmaceutical care is considered an asset (e.g.) Accredited Canadian Pharmacy Residency, Board of Pharmacy Specialties, Certified Diabetes Educator, Academic detailing, critical appraisal, TEACH.

Group facilitation and/or program planning experience.

Familiarity with Electronic Medical Records (EMRs) such as Telus PS Suite, Accuro, Avaros, OSCAR
