Description complète du poste

The Case Manager is responsible for providing the highest degree of quality care and services by providing clients and families with the highest-level of service. They will assist clients in navigating their daily lives while in the care of Together We Can. They must have the ability to show empathy and compassion towards all participants. Case Managers must have an understanding of and are responsible for helping to guide clients through their recovery, Indigenous cultural healing, 12-step and SMART recovery models.

What you will do

Complete Intake assessments and mid-term and aftercare reviews in a thorough and detailed manner as needed.
Ensure client needs are being met and addressed by meeting 1:1 weekly
Case manages clients and establishes a point of contact for families and third parties
Interact courteously and tactfully with clients, co-workers, family members, visitors, third-party funding sources and the public
Communicate effectively with funding providers and ensure the Society, participants, and funding providers are all working collaboratively in the client’s best interest
Resolve conflicts and provide crisis intervention
Debrief to Supervisors as required
Communicate effectively as a member of the Counselling Team
Document communication with clients through case and journal notes
Continue to grow professionally and develop personal standards
Maintain the Standard of Care and professionalism as set by the society
Maximize retention of clients
Ensure treatment duration is in line with agreed-upon timelines and that intake/discharge dates are accurate in FYI or Salesforce
Coordinate client’s needs with different departments and services
Document client interaction in FYI or Salesforce
Update management about any concerns with the client
Submit a report on the last day of the work week
Aftercare & Transition planning with participant
Send referrals to partner organizations or local services as needed
Maintain contact with graduates for up to six weeks following discharge

What you bring

Excellent verbal and written communication skills
Ability to work with compassion towards individuals who suffer from addiction issues
Basic phone/computer/electronics skills (email, Microsoft Word, Microsoft Excel, Internet, Fax, Copier, etc.)
Time Management & Organizational Skills
Conflict resolution
Ability to adapt quickly to changing priorities
Self-motivated
Ability to work under pressure
Ability to work as part of a team or individually

Skills and strengths

Strong attention to detail and the ability to maintain accurate records and follow clear processes
A high standard of conduct, sound work ethic, and flexibility in a dynamic environment
Basic computer, phone, and office equipment skills, including email, Microsoft Word, Microsoft Excel, internet use, fax, and copier functions
Effective verbal communication and listening skills
Basic conflict resolution skills and the ability to work cooperatively as part of a team
Compassion, professionalism, and a willingness to continue learning and growing through ongoing training and education

Pay: $26.00-$28.00 per hour
