Job description:
The patient care navigator will work with the team to assess current patient demand, analyze the Clinic's ability to meet the needs of patients across NWO and assist in developing a plan to offer improved patient care. This will be done by:
- analyzing trends in patient demand via the Janeapp appointment booking software program.
- meeting regularly with NDs and NPs at the Clinic to learn about their experiences in treating patients.
- meeting regularly with reception staff in order to understand how patients find out about the Clinic, which practitioners they are referred to, and how long they have to wait for services.
- assisting with new patient health assessments and researching patients' health care concerns.
- orienting new patients to the healthcare services that are offered at TBNC.
- helping to identify inefficiencies in the way that healthcare services are delivered at the Clinic.
- helping identify areas of learning for the TBNC team; ensuring that Indigenous cultural competency training is up to date, and that services are accessible to all people in northwestern Ontario.
- networking with local MDs, NPs to strengthen each patients' circle of care
- assisting with research and advocacy work for Indigenous Peoples https://www.tbaynaturopathic.ca/research
- outreach to TBRHSC cancer care team to increase mutual understanding of medical oncology care and naturopathic support for patients with cancer, and how to best integrate the two.
Qualifications
- Strong patient service and communication skills, including the ability to build trust, listen actively, and explain information clearly and respectfully.
- Experience in healthcare administration, patient coordination, or a similar role in a clinical or wellness setting is beneficial.
- Solid organizational and time management skills, with the ability to manage multiple appointments, tasks, and follow-ups while maintaining attention to detail.
- Proficiency with office software, electronic scheduling systems, and patient record management; comfort learning new digital tools and clinic systems.
- Knowledge of or interest in naturopathic medicine, integrative health, or wellness services, with a willingness to learn about clinic offerings.
- Demonstrated ability to work collaboratively in a team environment, maintain confidentiality, and handle sensitive information in line with privacy regulations.
- Post-secondary education in health sciences, naturopathic medicine or social services, medical office administration, or a related field.
- Availability to work full-time on-site in Thunder Bay, ON, with flexibility for clinic hours as needed.
