Clinical Care Coordinator | Long-Term Care | Vernon. BC

$37.00- $48.00/hour | Full-Time | Days

You support teams, coordinate care and improve lives. What Your Days Will Look Like

Collaborate with interdisciplinary teams
Support and mentor nursing and care staff in clinical practices
Support regulatory compliance, audits, and quality improvement initiatives
Monitor and evaluate care plans, ensuring they reflect residents’ changing needs
Assist staff with education, orientation, and ongoing training
Participate in admissions, discharges, and care conferences
Act as a resource for clinical decision-making and problem solving
Promote infection prevention and control practices
Investigate and follow up on incidents, concerns and clinical risks

What You Bring

Licensed Practical Nurse (LPN) and is good standing with BCCNM
5+ years nursing experience in a long-term Care settings
3+ years leadership experience in healthcare settings
Valid First Aide Certification
Strong clinical knowledge and leadership skills
Successful Criminal Record Check (vulnerable sector)
Understanding of care planning, assessments, and regulatory standards
Ability to mentor, coach, and support staff
Excellent communication, organization, and problem-solving abilities
Commitment to person-centered care and continuous improvement
